Well, this "sign" is marked on the Sarugaki Hiyori's gym suit.
It means simply "Sarugaki", yes it's her surname.
Sarugaki Hiyori is a character from Bleach and appears in the chapter 189 of the manga and in ep. 112 of the anime.
